Middleville AYSO Registration Information
Fall 2010/Spring 2011 Registration
As of September 1, 2010 we will no longer be accepting Fall registrations - Spring 2011 will continue to be accepted.

To register to to www.eayso.org and follow the screen prompts. Once you have hit the “I Agree” button, you will need to find the “PRINT” button, and print out 2 copies, sign and date both copies, write the child uniform size at the top (ex. YS, YM, AS, AM.. ect).
Please send both copies and a check for $85 to: Attn: Registrar, Middleville AYSO, PO BOX 430, Middleville, MI 49333. Your child will not be placed on a team until BOTH copies of the forms are received.
To be eligible to participate in Middleville AYSO 10/11, players must be born on or before July 31, 2006, (making them at least 4 years old before the beginning of the season) and they cannot be born before August 1, 1996. (i.e. 13 years or under at beginning of season).
Ages 4 through 9 play their games here in Middleville on our fields behind the TK Middle School. Players age 10-13 travel to other regions for approximately half of their games.
The registration fee includes a team jersey, shorts, socks, insurance, and six games in the fall and 6 games in the spring. |
